Dear Neurosurgical Colleagues,

It is our honor to announce the establishment of the Walter E. Dandy Neurosurgical Society. The mission of this new society is to be the global society for neurosurgeons representing education, techniques, and innovations in operative neurosurgery.

We would like to take this opportunity to outline some of the key initiatives that this society will provide to its members:

• Society Certification in Neurological Surgery: The society will provide a number of modules that encompass the various subspecialties of neurosurgery. For those members who wish to take these modules and pass each subsequent examination, the society will issue a certificate acknowledging the member has met the society’s criteria for core knowledge in Neurological Surgery. The modules will be offered at various international locations.
• Online Case Consultation: Members will have access to online forums where they will be able to upload relevant patient information and diagnostic imaging to receive advice from neurosurgical colleagues and experts from around the world.
• Monthly Summary of Neurosurgical Literature: Each month the WEDNS will electronically distribute to its members a synopsis of those recently published journal articles that are most relevant to the practicing neurosurgeon. Click here to view the sample from March 2012.
• Online Operative Video Atlas: Members will have access to an online catalog of intraoperative case videos organized by disease to aid in surgical planning.
• Dandy College of Neurosurgery: For those members that meet strict criteria, there will be an opportunity to become a Fellow of the Dandy College of Neurosurgery in a particular subspecialty. Criteria include review of practice data, patient outcomes, and an oral examination.

Em geral, os médicos ainda se “aventuram” pouco pelo mundo dos investimentos. O mau relacionamento entre estes profissionais e os investimentos vem de muitos séculos. De fato, a maioria dos médicos sabe muito bem como ganhar dinheiro mas sabe muito pouco como cuidar do dinheiro que ganham. Usando as palavras do Dr. Maurício Hissa ao prefaciar o livro Bolsa de Valores para Médicos (Editora DOC) – “Doutor! Doutor! Estuda tanto, salva vidas, sabe cada detalhe do corpo humano mas, na hora de cuidar do seu dinheiro duramente ganho nas noites mal dormidas e na pressão do minuto que salva uma vida, parece que voltou aos tempos da sangria e dos cirurgiões barbeiros.”

Investimento em ações para médicos

No entanto, ao longo dos últimos dois séculos, a cirurgia tornou-se radicalmente mais eficaz, e sua violência substancialmente reduzida – alterações que se revelaram fundamentais para o desenvolvimento de habilidades da humanidade para curar os enfermos.

A hospital in Texas, Citizens Medical Center, has said that it is not going to hire anybody — doctors, health staff, nurses — who is overweight. For them, that means a body mass index of over 35 kg/m²; or in other words, for a 5’10″ man, if you weigh more than about 250 lb, you wouldn’t get hired at this particular Texas facility.Today’s topic is one that I think will get everybody’s attention. To put it bluntly, should hospitals hire employees who are overweight?

So, just like people are talking about not hiring employees who smoke at hospitals or nursing homes or clinics, this institution is the first that I know of to say, “We are not going to hire overweight people.”

Now, you might think this is illegal, but it isn’t. It turns out that there are no laws against discrimination against overweight people except in Michigan, and a few cities have passed these laws.

Maybe overweight can be treated as a disability, but you would have to go to court and fight that, and I’m not sure how it would turn out. So, if the hospital wants to try this in Texas, it probably can without violating any civil rights statutes.

What is the argument that they have for doing this? They say, “Look, if you are going to come in and counsel people about health, you need to have a staff that is slim and fit.” It is certainly the case that it is hard to have people say, “Follow what I say, don’t do what I do,” and I think they are worried that it might not be as attractive to patients to have someone tell them to lose weight who is overweight themselves.

It is also the case that overweight staff sometimes require their own special equipment and furniture, so there can be costs there, and it turns out that overweight people may be absent more just because of health-related problems. There is probably some argument that it raises healthcare insurance costs because of the problems associated with being overweight.

So, maybe there is a case here, but let’s look at the pros and cons. On the side of doing this, it may be good for patients and encourage them, maybe it will cut down the costs for the hospital, maybe it will send a message that we expect our staff to maintain good health habits themselves as part of trying to encourage it in others.

What’s on the negative side? I can think of a major problem, and that’s treating healthcare risks equally. If you want to go after overweight, then who is going to sit in the hospital parking lot and see who is speeding when they come in? Who is going to make sure that someone arriving on a motorcycle or a bicycle is wearing a helmet? Who is going to make sure that they are wearing their seatbelts when they come to work? And to take this a bit further, in Texas, who is going to make sure that they are not riding horses at home because it is dangerous; or own a gun, which turns out to be a big health risk? There are a lot of other equally risky things besides weight that doctors or nurses or healthcare staff might do, and the question is, are we going to control that?

Look, I’m all for trying to set a good example and I think there are plenty of businesses where being thin and being in shape really do matter. I guess if you run a modeling agency it is very important. But I’m not convinced, really, that putting in weight restrictions is the best idea in terms of sending out the right message or a necessary message to patients. Patients, I think, can work with their doctors to try to overcome common problems. Doctors see all kinds of patients with all kinds of habits and all kinds of lifestyles. I think patients can deal with seeing all kinds of healthcare workers with all kinds of habits and all kinds of lifestyles. If they want a thin one, they should be able to pick one, but I don’t think the hospital necessarily should have to say that only the thin ones can work here.

Papel do Neurocirurgião no Sistema Integrado de Saúde (EUA)

O governo vai investir R$ 15 milhões em pesquisa e produção de células-tronco em escala comercial, segundo informações do Ministério da Saúde. Cerca de R$ 8 milhões serão destinados à conclusão de oito centros nacionais de terapia celular, enquanto os outros R$ 7 milhões serão aplicados em pesquisas na área. Os editais para esses devem ser publicados ainda em 2012.

A ideia do governo é ampliar o uso terapêutico das células-tronco em pacientes da rede pública de saúde, como em casos de recuperação do coração, movimento das articulações e tratamento de esclerose múltipla. Outro objetivo é dar condições aos centros nacionais de produzir quantidade suficiente de células para abastecer hospitais públicos e particulares, que dependem, na maioria dos casos, de material importado.

Três centros dos centros nacionais de terapia já funcionam – em Curitiba (PR), Salvador (BA) e Ribeirão Preto (SP). O trabalho mais avançado é o da Pontifícia Universidade Católica (PUC) do Paraná, que já tem células-tronco adultas e aguarda o aval da Agência Nacional de Vigilância Sanitária (Anvisa) para produzir em escala comercial.

O uso de células-tronco derivadas da medula óssea apresenta bons resultados no tratamento de doenças no sangue, como leucemia e anemias, de acordo com o Ministério da Saúde.

Uma estrutura neural chamada parecida com “um favo de mel” pode ajudar nervos danificados a crescer e se recuperar futuramente, aponta uma pesquisa publicada no jornal Biofabrication, especializado na área. A estrutura pode reunir várias ramificações de um nervo por seus poros, eventualmente reparando o nervo. Os cientistas acreditam que, futuramente, será possível curar lesões na espinha dorsal com o “favo de mel miniatura”, também chamado de andaime.

Quando os nervos sofrem danos, como ocorre comumente em acidentes de carro, o corpo pode perder o movimento de membros ou a sensibilidade de determinadas regiões. A restauração do nervo pode ser um desafio, mas se ele estiver fora da espinha dorsal ou do cérebro, pode se curar sozinho, contando que não tenha sido muito prejudicado.

Uma das técnicas que ajuda o reparo dos nervos é o uso de tubos. As duas pontas do nervo são colocadas dentro da estrutura, que vai “guiar” as partes da célula a se unirem novamente.

Os pesquisadores da Universidade de Sheffield (Grã-Bretanha) e do Laser Zentrum de Hannover (Alemanha) estudara a restauração de nervos usando a estrutura do favo de mel. O doutor Frederik Claeyssens, da instituição britânica, afirmou que o andaime é “bem parecido com a estrutura do nervo”. “O nervo tem pequenas regiões onde há ‘cabos’ que vão de um lado ao outro, e eles estão dentro de uma estrutura maior. É isso o que queremos reproduzir com o andaime”, explicou.

A estrutura introduzida pelos cientistas é feita de ácido polilático fotopolimerizável, que se desfaz depois que o nervo se repara automaticamente. Os pesquisadores mostraram que células neurais podem se desenvolver junto com o favo e agora estão conduzindo testes para tentar fazer com que nervos sejam completamente restaurados.

Segundo Claeyssen, essa tecnologia “pode fazer uma grande diferença para pacientes que tiveram nervos danificados”. A estrutura do favo já é usada em outras áreas da medicina.
